Primary Care of WNY Recognized for Early Detection of Patients’ Palliative Care Needs

Palliative Care Buffalo and Hospice Buffalo are proud to recognize their partnership with Primary Care of Western New York (WNY) to provide quality healthcare for patients and families facing challenges with serious or end-of-life illness.
David Martinke, Medical Director of Primary Care of WNY, was honored to receive Palliative Care Buffalo’s “Partners in Caring” plaque, “I would like to express appreciation to Hospice Buffalo for providing quality and compassionate care for the patients of our practice and for the community of WNY.”
Dr. Kerr, Hospice Buffalo’s CEO and Medical Officer, presented “Partners in Caring” plaques to each of Primary Care of WNY’s three locations: Williamsville, Kenmore and Wheatfield [see photo enclosed].

Special thanks to the Care Nurses for earlier identification of patients with chronic and advancing illness who need Palliative Care and support.”

Care collaboration focuses on managing pain and symptoms, reducing Emergency Room visits, and improving the quality of life for patients and their families. Palliative Care is available to anyone with serious, progressive or chronic illness with active symptoms, who may still be receiving curative treatments. It does not require a limited prognosis and can begin at diagnosis or any time during the disease.
