In addition to the quality care and support services that we provide, Hospice & Palliative Care Buffalo offers a connection to valuable community resources that can help patients and families further manage the stress and complexities of living with serious illness.

Resources for healthcare providers

Our clinicians and outreach staff can provide education and resources for you, your team and your patient community. Also, upon your request, we can spend time with a patient and family in follow-up to your conversations about disease progression and goals of care.
Many people think hospice and palliative care are one in the same, yet they are two different and distinct care options. Both palliative care and hospice care focus on helping to manage symptoms and improve quality of life, but hospice can only begin when it is felt that the individual has a prognosis of six months or less and curative treatment of the disease has stopped; palliative care can begin much earlier, even at the time of diagnosis and while receiving treatment.
Hospice & Palliative Care Buffalo is here to help educate you and your family about advance directives and advance care planning. We can make difficult conversations easier by guiding you through the process of sharing your health care wishes and preferences with loved ones and your health care providers.

The Hospice & Palliative Care Buffalo research team was established to better understand – and educate – professionals and communities about the impact of hospice and palliative care.
