Hospice & Palliative Care Buffalo (HPCB) cares for individuals suffering from serious illness, as well as their families and caregivers. We provide an integrative, holistic (physical, emotional, social, and spiritual) approach to care that focuses on the whole person rather than on individual symptoms. We are here through a continuum of care, spanning the course of the disease through bereavement.
|Hospice Buffalo cares for seriously ill people with a prognosis of 6 months or less to live. Each patient receives customized, comprehensive care from an interdisciplinary team of healthcare professionals: physician, nurse, social worker, grief support counselor, and volunteer.|
|Home Connections - adult palliative care - provides home-based, case-managed care and support to patients facing serious, progressive illness and their families.|
|Essential Care for Children is a home-based palliative and hospice care program that provides coordination of medical services, as well as psycho-social, emotional and spiritual support to seriously ill children and their families.|
|Grief support and education that is sensitive, compassionate and responsive to the needs of the WNY community. Services are provided by licensed healthcare professionals and peer volunteers.|
