Here with Care – Expertise and Comfort in Area Hospitals
Palliative Care in Hospitals
Palliative care is available in most area hospitals as a short-term solution when pain or symptoms cannot be managed at home. Our care team focuses on comfort, care, assistance and support for both the patient and family. We work closely with hospital staff to coordinate care and continually evaluate and alter care plans to ensure your loved one is comfortable. We assist with the management of uncontrolled symptoms, especially pain, prior to discharge.

Hospital Nurse Liaisons
Hospice Buffalo Nurse Liaisons have a presence at all area hospitals to identify patients who would benefit from receiving palliative or hospice care. They work closely with the hospital’s palliative care team and healthcare providers to evaluate your loved one and coordinate an ideal plan of care. Expert coordination of care and symptom management by trained healthcare providers may shorten the hospital stay. Our Hospital Nurse Liaison can also help ease the transition to home or a long-term care setting.
