Hospice Buffalo Nurse Liaisons have a presence at all area hospitals to identify patients who would benefit from receiving palliative or hospice care. They work closely with the hospitals palliative care team and healthcare providers to evaluate your loved one and coordinate an ideal plan of care. Expert coordination of care and symptom management by trained healthcare providers may shorten the hospital stay. Our Hospital Nurse Liaison can also help ease the transition to home or a long-term care setting.
