Position:  RN Primary Case Manager
Job Type:  Full Time: Days 8:00 AM - 4:00 PM
Description: 

Graduate of an accredited school of nursing & licensed to practice in NYS. Certification in Hospice & Palliative Care Nursing encouraged. A minimum of 2 years of nursing practice as a professional nurse is preferred. Must have a current NYS drivers license & a car available for use when on duty or on-call.
