Optimum Care Family

The primary goal of Optimum Care is to maximize your quality of life.

Having a serious illness can be overwhelming. The Optimum Care Program brought to you by Hospice Buffalo can help. Optimum Care is a home care program that allows individuals to:

  • Stay in their own home
  • Maximize their quality of life
  • Reduce stress on loved ones
  • Eliminate or reduce the need for ER visits or hospitalization by managing symptoms in the home

The primary goal of Optimum Care is to maximize your quality of life.

  • Modern treatments for heart failure slow but do not stop progression of the disease

  • Hospice patients live longer than similar patients who do not choose hospice care

What Services Does Optimum Care provide?

  • 24 Hour Access to your team: A nurse, physician, social worker, chaplain and others.
  • Customized care to achieve your goals.
  • Ongoing communication between you, your doctor, your family and your hospice team to address all aspects of your care - medical, emotional and spiritual.
  • Symptom control and consultation by the hospice medical team when requested.
  • Disease specific instruction to manage your illness at home, and what to do if a problem occurs.
  • Caregiver support and instruction to assist family members.
  • Freedom from medical bills. Any medication or durable medical equipment (ex. oxygen, wheelchairs, etc.) related to your primary diagnosis is covered in full by Medicare and most insurance providers. This includes delivery right to your home.
  • Assistance from Home Health Aides and Volunteers (based on need) with personal care, home making and shopping.

  • Multiple emergency room visits
  • Hospitalizations over the past 6 months
  • Recent decline in ability to do daily tasks
  • Unintentional weight loss over the prior 6 months
