 Individual & Family Counseling

Our professional staff is available to provide individual and family counseling for people of all ages who need help coping with the death of a loved one or who are struggling with the effects of chronic or life-threatening illness. We accept all major insurances.  If your insurance will not pay for counseling, a sliding fee scale, based upon income, will be used to determine your fee.

Support Blue

A specialized home care program that coordinates services between you, your doctor, and your insurance. It is designed for people with serious illness and their loved ones.

Advance Care Planning

Advance Care Planning combines emotional support and assistance with long term planning for individuals who are diagnosed with a serious, potentially life-threatening illness.

Crisis Intervention/Postvention

Life Transitions Center provides emergency support following a death in a school, business or agency. Professional staff is available to offer on-site assistance when a school or place of work is struggling to support a student or employee who is seriously ill.
