Assisted Living and Nursing Home Services 
Improving Quality of Life for Residents and Their Families
While many Hospice Buffalo participants receive care in their homes, the Hospice assisted living and nursing home program enables us to bring our particular expertise to residents of area facilities. The program's purpose is to allow residents in the advanced stages of illness to remain in familiar and supportive surroundings while receiving palliative (comfort-focused) care. Hospice enhances the specialized expertise of the nursing assisted living and home team with the special care, emotional support and guidance residents and their families need during this distressing time.
This program is available for any Erie County Hospice eligible patient, in facility, with advanced, end-stage illness. The following guidelines may indicate the need for the program:
• Frequent doctor office visits with related complications
• Frequent emergency room visits or hospital admissions in the past year
• Deteriorating functional or physical status
Hospice Buffalo serves more than 40 assistewd living and assisted living and nursing homes in Erie County.

Admission is based on medical need, not the ability to pay. Insurance coverage is available through most third party insurers, including Medicare and Medicaid.
Hospice Buffalo has agreements with all major area hospitals to accept Hospice assisted living and nursing home residents if more intensive medical care becomes necessary. In-facility informational evaluation by a Hospice RN to determine the needs of the patient and family is free of charge.

Hospice Specialty Programs
Family Matters
This new program helps to keep you informed, prepared for upcoming changes and supported through the course of your loved one's illness. Hospice's specially-trained social worker can meet with family members during the evening. In addition, Family Matters offers palliative care (comfort-focused) and a Hospice consult by a Hospice physician.
Life Rhythms
A new music therapy program hosted by Hospice Buffalo's certified music therapists. Life Rhythms facilitates interaction, participation, and can help families reconnect.
One local Activities Director summed up Life's Rhythms in this way:
"In the 13 years I've worked in activities, this is by far the most vital program I've seen. The residents are stimulated and engaged, and respond at an amazing level. It is beautiful to see. Thank you."
