What is Caring Hearts Home Care?

Caring Hearts Home Care is a licensed home health care service agency licensed by the New York State Department of Health. We provide home care services to the Western New York community. Our skilled personnel are licensed, registered professionals and are fully insured.

As an affiliate of The Center for Hospice & Palliative Care, you can be assured of receiving the highest quality of care from our staff.

Home health care provides health care services in the comfort, convenience, and privacy of your own home.

Our special affiliation with Home Care Buffalo and Hospice Buffalo allows patients to be transferred easily into a Certified Home Health or Hospice program if it becomes desired or appropriate.

What Services Does Caring Hearts Home Care Provide?

Caring Hearts Home Care, under the direction of your physician and in cooperation with you and your family, will establish a plan of care specifically designed to meet all your needs. We provide:

• Certified home health aides
• Personal care aides
• Home assessments
• 24-hour on call availability
• Nursing by a registered nurse and licensed practical nurses
• Homemaker/Housekeeper - help with errands, grocery shopping, light housekeeping or personal laundry of patient
• Arrangements for medical equipment and supplies
• Arrangements for therapy services as needed

You or your family member, friend, or health care professional can call Caring Hearts Home Care at 686-8300, or email info@palliativecare.org. We respond to this call within hours and perform a free patient assessment.

Caring Hearts Home Care serves all people regardless of race, creed, color, handicap, sexual orientation, marital status, national origin, sex, age, diagnosis, or payment source.

Caring Hearts Home Care will coordinate with insurance plans for covered services.
